Every driver of a van or a lorry is supposed to check it before they set off, and the paper form in the cab says they did. Nobody can tell from the form whether the check was real, whether a defect was ever fixed, or whether the vehicle that failed on Tuesday went out on Wednesday. When DVSA ask, the form is all there is.

Questions That Fit the Vehicle
A van is not asked about air brakes or a tachograph. Trailer questions appear only when one is coupled. What does not apply is still recorded as not applicable, with the reason, so a check always shows the full picture.
The Inspector’s Own Wording
Above 3.5 tonnes the choices become Satisfactory, Monitor, Repair Required and Safety Item Defect, with the Inspection Manual reference beside each one.
Faults the Driver Cannot Talk Down
Brakes, steering, seatbelts, lights, wheels, load security and coupling are always the most serious category if they fail. A dangerous defect grounds the vehicle and the manager is told at once; only a manager can clear it.
Nothing Is Forgotten
Outstanding defects carry forward to that vehicle’s next check, whoever is driving it.
Evidence, Not Just Ticks
Photos and video, a tap-on diagram of the vehicle for damage, the odometer, the location and time, and one signed declaration.
Trailers as Their Own Records
Custom trailer types with their own questions. A trailer grounds separately from the vehicle towing it.
Faults Found on the Road
A driver can report a fault mid-shift with one tap and their location. “Not driveable” grounds the vehicle the same way a failed check does.

The Driver’s Own Journal
A private diary with photos, a tachograph printout or a delivery note, locked after six hours. Nobody else can read it, including the manager.
A Record That Stands
Every completed check produces a PDF report. Checks cannot be edited or deleted once submitted.
Works in the Yard
Installs to the phone. If the signal drops, the check and its photos are queued and sent when it returns.
